APPENDIX I - LABEL REPORTING

Columbus Mobility Specialists Inc. will complete a monthly report of labels used and labels on hand in the frequency and format described in Appendix I (this page), Appendix J, Appendix K and Appendix L.  The form in Appendix M shall be used to order more labels as needed.  For reporting purposes, the reporting month is the calendar month.

The following reports shall be sent to RADCO each month:

• One copy of the form in Appendix J (NMEDA Montly Label Use Form).  Multiple pages may be submitted if they are needed to list all of the NMEDA labels used in the month.
  

• One copy of the form in Appendix K (NMEDA Monthly Use Form Summary).
  

• One copy of the form in Appendix L (NMEDA Label Use Form) for each NMEDA label applied within the month.  This form is the basis for the accounting of NMEDA labels.

NOTE - Accuracy and completeness are extremely important, as these forms will provide the basis for the statistical equipment report, which will be provided by RADCO to NMEDA.

The reports will be sent to reach RADCO by the 10th of the month following the reporting month. It will be mailed to:
