4.0 NMEDA LABEL

Section 4 deals with NMEDA label installation, label record keeping, and label reporting.

4.1 LABEL INSTALLATION

Columbus Mobility Specialists Inc. will install a serially numbered NMEDA approved label on each modified vehicle, on the driver's side doorpost. Columbus Mobility Specialists Inc. label installation instructions are in Appendix H.

4.2 LABEL RECORDS

Columbus Mobility Specialists Inc. will maintain a record of all NMEDA labels installed, including:

- Label number

- Vehicle description

- Customer name

- Customer address

- Date label is affixed

- Description of modifications and equipment / components installed

RADCO will verify labels used, and labels on hand, during each audit.  Label records will be kept for a minimum of 5 years.

4.3 LABEL REPORTING

Columbus Mobility Specialists Inc. will complete the NMEDA Label Reporting Form (Appendix J) for each vehicle labeled during the preceding month, and transmit it monthly to RADCO headquarters, with a completed NMEDA Monthly Label Use Form (Appendix K).  See Appendix I for further details. Label reports will be kept for a minimum of 5 years.
